Aims

To provide delegates with a range of simple systematic techniques that will ensure a consistent approach is adopted when investigating incidents and accidents.

Objectives

- Know the company reporting procedures relevant to their business
- Value the importance of pre-investigation planning
- Know the theory of accident causation
- Understand how to investigate the accident scene
- Interview victims and witnesses
- Analyse data  and establish root causes
- Know the essential elements of the accident report

- Defining the accident/incident
- The moral, legal and financial imperatives for  investigations
- Accident causation models
- Human factors in accident causation
- The investigation process
      
    - Stage 1 - preparation

    - Stage 2 - the investigation

    - Stage 3 - research and analysis

    - Stage 4 - the report
